Background:

  • Synchronous upper and lower urinary tract tumors are a clinical challenge.
  • The da Vinci Surgical System is an evolving platform for urological procedures.

Purpose of the Study:

  • To report the first robotic case series of combined complex upper and lower urinary tract surgery.
  • To evaluate the safety, feasibility, and outcomes of concurrent robotic procedures.

Main Methods:

  • Six high-risk patients (median age 71) underwent concurrent robotic surgery.
  • Procedures included robotic nephroureterectomy with robotic-assisted radical cystectomy (RARC) or radical prostatectomy (RALP).
  • Intra-corporeal ileal conduit urinary diversion was performed in four patients.

Main Results:

  • Mean length of stay was 10 days; average blood loss was 416.7 mL.
  • No Clavien grade 3-5 complications or 30/90-day mortality occurred.
  • Clear surgical margins were achieved in all patients.

Conclusions:

  • Combined upper and lower urinary tract robotic surgery is safe and technically feasible.
  • The procedure demonstrates acceptable complication rates and oncological outcomes.
  • Robotic surgery offers a viable option for managing complex, synchronous urinary tract pathologies.
