Early HIV infection causes nerve damage. A two-year study found progressive spinal cord and peripheral nerve dysfunction in individuals with HIV antibody but without AIDS, suggesting a direct viral effect.

Background:

  • Delayed nerve conduction is observed in patients with Acquired Immunodeficiency Syndrome (AIDS).
  • The cause of this neurological dysfunction in relation to Human Immunodeficiency Virus (HIV) infection or immunodeficiency remains unclear.

Purpose of the Study:

  • To investigate whether delayed nerve conduction in HIV-positive individuals without AIDS is linked to HIV infection itself or the resulting immunodeficient state.
  • To characterize the progression of neurological changes in early-stage HIV infection.

Main Methods:

  • A two-year prospective follow-up study involving electrophysiological measurements.
  • Twelve homosexual men positive for HIV antibody but without AIDS were monitored.
  • Eight specific nerve conduction latencies were measured, including spinal and peripheral pathways.

  • Spinal latencies showed a mean increase of 4.2% at C7 and 5.5% at T12.
  • Conduction time from the gluteal crease to T12 significantly increased by 32.0%.
  • Peripheral nerve conduction (median and tibial) showed milder increases of 5.6% and 2.2%, respectively.

Conclusions:

  • Early HIV infection is associated with both peripheral neuropathy and myelopathy/myeloradiculopathy.
  • These neurological deficits appear to progress slowly and may result from a direct neurotropic action of HIV.
  • The findings suggest that neurological complications can occur even before the development of AIDS.
