Observation:
- A 71-year-old male presented with intractable productive cough, dyspnea, and hypoxemia unresponsive to asthma treatment.
- Diagnostic findings included marked peripheral blood and sputum eosinophilia, bilateral centrilobular nodules on CT, and mixed ventilatory impairment.
Findings:
- Clinical diagnosis of eosinophilic bronchiolitis was made due to progressive respiratory failure precluding invasive procedures like lung biopsy.
- Prompt administration of oral prednisolone resulted in rapid symptomatic and objective improvement.
Implications:
- Eosinophilic bronchiolitis should be considered in the differential diagnosis of patients with persistent obstructive lung disease and eosinophilia.
- Sputum eosinophil counts may serve as a valuable non-invasive biomarker for diagnosing eosinophilic bronchiolitis, especially when advanced investigations are contraindicated.
