Convexity subarachnoid hemorrhage (cSAH) can be an early complication of acute ischemic stroke. This study reports three cases, highlighting cSAH as a potential diagnosis in stroke patients.

- No evidence of cerebral amyloid angiopathy was found on MRI-GRE.
- All patients presented with ipsilateral acute ischemic stroke.
- Patients remained clinically stable with minimal neurological deficits at discharge.
Implications:
- These findings add to the literature linking cSAH with ischemic strokes.
- cSAH should be considered in the differential diagnosis of acute ischemic stroke.
- Early recognition and management of cSAH as a complication of stroke may improve patient outcomes.
