Summary

ERG immunohistochemistry (IHC) offers limited diagnostic value when combined with AMACR-CK5/6 for prostate needle biopsies. ERG expression in prostate cancer is linked to lower Gleason scores and higher tumor volume.

Background:

  • Prostate needle biopsy interpretation relies on accurate diagnosis of prostate carcinoma (PCA).
  • Immunohistochemistry (IHC) aids in differentiating benign mimics from malignant glands.
  • ERG and AMACR-CK5/6 are established IHC markers in prostate pathology.

Purpose of the Study:

  • To evaluate the diagnostic and prognostic utility of ERG IHC when added to AMACR-CK5/6 staining in prostate needle biopsies.
  • To assess the association of ERG expression with clinicopathological features of PCA.

Main Methods:

  • 119 prostate needle biopsies with AMACR-CK5/6 IHC and 16 high Gleason score cases were analyzed for ERG expression.
  • IHC results were correlated with diagnoses including PCA, high-grade prostatic intraepithelial neoplasia (HGPIN), and benign mimics.
  • Clinicopathological parameters such as Gleason score (GS), tumor volume, and bilaterality were recorded.

Main Results:

  • ERG was detected in 36% of PCA, 27% of HGPIN, and 13% of atypical foci, but not in benign mimics.
  • ERG-positive HGPIN showed a strong association with concurrent ERG-positive PCA in the same core.
  • ERG expression in PCA was inversely related to GS and trended towards association with higher tumor volume and bilateral disease.

Conclusions:

  • ERG IHC provides limited additional diagnostic value when used alongside AMACR-CK5/6.
  • ERG expression in PCA correlates with less aggressive features and higher tumor burden.
  • ERG-positive HGPIN may indicate an increased risk for subsequent PCA development, warranting further prognostic investigation.
