Background:

Purpose of the Study:

  • To investigate the association between consanguinity and neonatal death in rural Iran.
  • To identify familial marriage as a risk factor for neonatal mortality.

Main Methods:

  • A nested case-control study was conducted in rural areas of Kohgiluyeh and Boyerahmad Province, Iran.
  • 6900 newborns were followed, with 97 neonatal death cases and 97 controls selected using risk set sampling.
  • Conditional logistic regression was used to estimate crude and adjusted odds ratios.

Main Results:

  • Consanguinity (first cousins) was significantly associated with neonatal death (OR = 5.23).
  • Other significant risk factors included prematurity (OR = 5.57), low birthweight (OR = 7.68), C-section (OR = 7.27), birth rank > 3 (OR = 6.95), and short birth intervals (OR = 4.65).

Conclusions:

  • Familial marriage to first cousins is a significant risk factor for neonatal death, even after adjusting for other variables.
  • Public health interventions should consider consanguinity as a modifiable risk factor to reduce neonatal mortality in the studied region.
