Summary

Salvage partial laryngectomy offers effective treatment for recurrent laryngeal squamous cell carcinoma (LSCC), preserving function. Transoral laser surgery (TLS) and supracricoid laryngectomy (SCL) show promising local control rates for selected patients.

Area of Science:

  • Otolaryngology
  • Surgical Oncology

Background:

  • Nonsurgical therapies are increasingly used for laryngeal squamous cell carcinomas (LSCC).
  • Surgical salvage strategies are crucial for managing LSCC recurrences after initial treatment.
  • Partial laryngeal surgery offers a functional and oncologically effective alternative to total laryngectomy for selected recurrences.

Purpose of the Study:

  • To critically review available studies on partial laryngeal surgery for recurrent LSCC.
  • To evaluate the oncological efficacy and functional preservation of salvage partial laryngectomy techniques.
  • To define the role of different surgical approaches in managing LSCC recurrences.

Main Methods:

  • Critical review of existing literature on partial laryngeal surgery for recurrent LSCC.

  • Analysis of pooled local control rates from multiple studies for transoral laser surgery (TLS) and supracricoid laryngectomy (SCL).
  • Evaluation of neck dissection indications in recurrent LSCC.
  • Main Results:

    • Salvage transoral laser surgery (TLS) provides tumor-tailored excisions with a pooled local control rate of 57% after the first procedure.
    • Supracricoid laryngectomy (SCL) demonstrates good local control (85% no recurrence) in selected supraglottic-glottic recurrences after radiotherapy.
    • Total laryngectomy after failed SCL achieved an overall local control rate of 65%.

    Conclusions:

    • Partial laryngeal surgery, including TLS and SCL, is a viable option for selected LSCC recurrences, preserving laryngeal function.
    • Neck dissection is mandatory for recurrent LSCC with neck metastases and recommended electively for supraglottic and transglottic recurrences.
    • Careful patient selection is key to successful oncological and functional outcomes in salvage laryngeal surgery.
