Intrapulmonary recombinant factor VIIa for diffuse alveolar hemorrhage in children

Intrapulmonary recombinant activated factor VII (rFVIIa) shows promise for treating diffuse alveolar hemorrhage (DAH) in children. This approach led to sustained hemostasis and improved oxygenation in pediatric patients with DAH.

- Retrospective review of 6 pediatric patients with bronchoscopically confirmed DAH treated between 2011 and 2013.
- Treatment involved intrapulmonary rFVIIa combined with methylprednisolone, fresh-frozen plasma, and platelet support.
- Patients had diagnoses including acute myeloid leukemia, myelodysplastic syndrome, hemophagocytic lymphohistiocytosis, T-cell lymphoblastic lymphoma, and idiopathic pulmonary hemosiderosis.

- All 6 patients achieved complete and sustained hemostasis without adverse events.
- Significant improvement in oxygenation (PaO2/FiO2 ratio) and rapid clinical recovery were observed.
- Four out of six patients experienced long-term survival and were successfully weaned from ventilators.

- Intrapulmonary rFVIIa appears to be an effective and safe treatment for pediatric DAH.
- The treatment facilitated rapid clinical improvement and enhanced survival rates in this cohort.
- Further clinical studies are warranted to confirm these findings and establish optimal protocols.
