Summary

Neck CT can help detect diffuse thyroid disease (DTD) incidentally. A classification of three or more abnormal CT features showed high accuracy in diagnosing DTD, aiding in preoperative assessment.

Background:

Purpose of the Study:

  • To evaluate computed tomography (CT) features of incidentally detected DTD.
  • To assess the diagnostic accuracy of CT for DTD.
  • To establish CT criteria for DTD detection.

Main Methods:

  • Retrospective analysis of preoperative neck CT scans from 209 patients undergoing thyroidectomy.
  • Evaluation of CT features including attenuation, gland size, margin, and enhancement.
  • Comparison of CT findings with histopathological results.
  • Receiver operating characteristic (ROC) curve analysis to determine optimal CT criteria.

Main Results:

  • Histopathology revealed normal thyroid in 157, Hashimoto thyroiditis in 17, non-Hashimoto lymphocytic thyroiditis in 34, and diffuse hyperplasia in 1 patient.
  • CT features suggestive of DTD included low/inhomogeneous attenuation, increased size, lobulated margin, and inhomogeneous enhancement.
  • A classification of '3 or more' abnormal CT features yielded sensitivity of 55.8%, specificity of 95.5%, and accuracy of 85.6% for DTD detection.

Conclusions:

  • Neck CT is a valuable tool for detecting incidental diffuse thyroid disease.
  • The presence of three or more abnormal CT features is a reliable indicator for DTD.
  • CT findings can aid in the preoperative diagnosis and management of DTD.
