Summary

A rare giant sacral perineurial cyst caused sciatic pain. Computerized tomography (CT) better visualized the cyst and sacral erosion than myelography.

Background:

  • Sacral perineurial cysts are typically small, asymptomatic nerve root cysts.
  • Large or symptomatic cysts are uncommon, rarely causing radicular symptoms.

Observation:

  • A rare case of a giant sacral perineurial cyst presenting with sciatic pain is described.
  • The cyst was associated with significant sacral erosion.

Findings:

  • Myelography provided poor visualization of the large cyst due to its size.
  • Computerized tomography (CT) offered superior definition of the cyst and associated sacral erosion.

Implications:

  • This case highlights the diagnostic utility of CT scans for large sacral perineurial cysts.
  • It underscores the importance of considering imaging modalities that best delineate complex sacral nerve root cysts.
