Area of Science:

  • Urology
  • Andrology
  • Surgical Innovation

Background:

  • Recurrent ischemic priapism presents significant clinical challenges, often necessitating invasive surgical interventions.
  • Conventional treatments for priapism may not always prevent recurrence or fully restore sexual function.

Observation:

  • A modified surgical technique combining a Winter's shunt with continuous cavernosal irrigation was applied to patients with recurrent ischemic priapism.
  • This technique was specifically used for acute ischemic priapic episodes in patients with a history of stuttering priapism.

Findings:

  • The combined technique successfully resolved priapism in all treated patients, with no reported recurrences.
  • Four patients demonstrated significant recovery of sexual function within six months of follow-up.

Implications:

  • This modified approach offers a less invasive and effective alternative for managing recurrent ischemic priapism.
  • The technique shows promise for improving patient outcomes, including the preservation of sexual function.
