Persistent polyclonal B cell lymphocytosis (PPBL) involves expanded B cells but shows normal proliferation and immunoglobulin secretion via CD40-CD154 activation. This suggests potential microenvironment or alternative pathway defects, not inherent B cell dysfunction.
