The knee joint is the most complicated joint in the body. It consists of three articulations– two tibiofemoral and one patellofemoral. As is characteristic of synovial joints, the knee joint has a thin articular capsule that partially surrounds this joint cavity. Background:

  • Total knee replacements (TKRs) utilize metal femoral and tibial components with a polyethylene insert as the bearing surface.
  • The surface finish of metallic components is critical for minimizing polyethylene wear and subsequent debris generation.
  • Understanding surface roughness is essential for improving the longevity and performance of TKRs.

Purpose of the Study:

  • To develop a protocol for characterizing the surface roughness of total knee prosthesis (TKP) metal components.
  • To assess the impact of wear on surface roughness parameters (Ra, Rsk, Rt) of TKP femoral and tibial components.
  • To correlate surface roughness changes with volumetric wear rates in TKPs.

Main Methods:

  • Six mobile TKPs (three size 2, three size 6) were subjected to wear testing on a knee joint simulator for 2 million cycles.
  • Polyethylene insert weight loss was measured using a gravimetric method to determine volumetric wear.
  • Surface roughness of metallic components was assessed at multiple points per component using parameters Ra, Rsk, and Rt.

Main Results:

  • Wear testing resulted in increased surface roughness for both femoral and tibial components across both TKP sizes.
  • No statistically significant differences in Ra, Rsk, or Rt were observed between size 2 and size 6 TKPs.
  • All metallic component surfaces exhibited a trend towards more negative skewness (Rsk), indicating diminishing peaks.

Conclusions:

  • Wear testing alters the surface topography of TKP metallic components, generally increasing roughness and reducing peak height.
  • The wear behavior and resulting surface roughness changes did not significantly differ between the tested TKP sizes.
  • Further investigation into the correlation between specific roughness parameters and wear debris generation is warranted.
