Summary

Internal suturing techniques for upper blepharoplasty significantly reduce suture abscesses and inflammation. This randomized controlled trial found fewer complications with internal versus external suture starts in eyelid surgery.

Background:

Purpose of the Study:

  • To compare the incidence of suture abscesses and focal inflammation between internal and external intradermal suturing methods in upper blepharoplasty.

Main Methods:

  • A Level I randomized controlled trial.
  • Upper blepharoplasty wounds were closed using either external or internal running intradermal sutures.
  • Assessment of suture abscesses, erythema, and edema at 1 and 6 weeks post-surgery.

Main Results:

  • After 1 week, 40.0% of externally sutured eyelids showed abscesses compared to 13.3% of internally sutured eyelids (p = .02).
  • Erythema and edema were also significantly lower in the internally sutured group at 1 week (p = .02).

Conclusions:

  • Starting the intradermal suture internally, rather than externally, significantly reduces medial wound inflammation and suture abscess formation in upper blepharoplasty.
  • The internal suturing method offers a potential improvement for managing complications in blepharoplasty wound closure.
