Summary

Nerve conduction studies show early sensory nerve damage in Hansen's disease (leprosy) patients, even before clinical symptoms appear. Further testing is needed to detect subtle nerve fiber changes in early leprosy.

Background:

  • Hansen's disease (leprosy) is a chronic, multi-organ illness.
  • Nerve involvement is a hallmark of leprosy, often preceding clinical manifestation.
  • Peripheral nerve damage can occur early in the disease process.

Purpose of the Study:

  • To evaluate nerve conduction parameters in thickened nerves compared to non-thickened nerves.
  • To assess nerve function in early tuberculoid leprosy.
  • To identify early electrophysiological changes in leprosy-affected nerves.

Main Methods:

  • Nerve conduction studies (NCS) including sensory and motor nerve conduction velocity (NCV), distal latencies, and amplitude.
  • Inclusion of 50 new, untreated male patients aged 15-50 with tuberculoid or borderline tuberculoid leprosy.
  • Comparison of thickened ulnar, common peroneal, and posterior tibial nerves with contralateral normal nerves.

Main Results:

  • Most thickened nerves exhibited deranged nerve conduction parameters.
  • Sensory nerve parameters were found to be affected early in the disease progression.
  • Early signs of nerve damage were observed in leprosy patients.

Conclusions:

  • Additional electrophysiological parameters may be necessary for assessing nerve damage in early leprosy.
  • Slow-conducting nerve fibers are affected early, but changes in overall conduction velocity may be subtle.
  • Separate measurement of fast nerve fibers is recommended due to the nature of recorded potentials.
