Summary

Stereotactic body radiotherapy (SBRT) offers effective tumor ablation for non-small-cell lung cancer and hepatocellular carcinoma. Further research is needed to determine optimal SBRT protocols and compare it with surgical options.

Area of Science:

  • Oncology
  • Radiation Oncology
  • Medical Physics

Context:

  • Stereotactic body radiotherapy (SBRT) involves image-guided, high-dose radiation for tumor ablation.
  • SBRT has demonstrated superior efficacy compared to 3D-CRT in clinical trials.
  • Established as a standard treatment for hepatocellular carcinoma (HCC) in unresectable cases.

Purpose:

  • To evaluate the effectiveness of SBRT in treating non-small-cell lung cancer (NSCLC) and HCC.
  • To explore optimal dose-fractionation schedules and reference methods for SBRT.
  • To compare SBRT with surgical interventions like sublobar resection.

Summary:

  • Phase II JCOG0403 trial reported 3-year overall survival rates of 76% for operable and 59.9% for inoperable T1N0M0 NSCLC patients treated with SBRT.

  • Ongoing phase II trial in Japan for primary HCC is expected to provide further evidence.
  • Limited enrollment in trials comparing SBRT with sublobar resection hinders definitive conclusions.
  • Impact:

    • SBRT is a viable and effective treatment modality for specific cancer types, particularly for patients unsuitable for surgery.
    • Further research will refine SBRT protocols, potentially improving patient outcomes.
    • Clarifying the optimal SBRT parameters will enhance its clinical application and standardization.
