Contrast-enhanced ultrasound (CEUS) shows promise in differentiating Leydig cell tumors from seminomas. Quantitative analysis of time-intensity curves (TICs) revealed significant differences in wash-in rate, peak enhancement, and time to peak.
Area of Science:
- Radiology
- Oncology
- Urology
Background:
- Ultrasound (US) is the primary imaging modality for testicular masses.
- Leydig cell tumors are typically benign (90% of cases), making accurate differentiation crucial.
Purpose of the Study:
- To evaluate the effectiveness of contrast-enhanced ultrasound (CEUS) in distinguishing Leydig cell tumors from seminomas.
- To utilize both qualitative and quantitative features of CEUS for differentiation.
Main Methods:
- Prospective multicenter study involving 31 patients with testicular lesions.
- All patients underwent grayscale US, color Doppler US (CDUS), CEUS, and ultimately orchiectomy.
- Paired one-tailed Student's t-test was used for statistical analysis.
Main Results:
- Hypoechoic lesions on grayscale US and atypical patterns on CDUS were observed.
- Qualitative CEUS analysis did not reliably differentiate between Leydig cell tumors and seminomas.
- Quantitative analysis of time-intensity curves (TICs) showed significant differences in wash-in rate (WiR), peak enhancement (PE), and time to peak (TTP).
Conclusions:
- Leydig cell tumors exhibit wider vascular beds and higher blood flow velocity than seminomas due to more regular neovascularization.
- Seminomas present with larger necrotic areas resulting from irregular neovascularization, explaining observed differences in PE and WiR.
- Further large-scale studies are needed to validate these preliminary findings.
