Background:

  • Group A Streptococcus (GAS) is a common pathogen, but GAS meningitis is a rare complication.
  • Understanding the epidemiology and clinical spectrum of GAS meningitis is crucial for patient outcomes.

Purpose of the Study:

  • To determine the incidence, clinical characteristics, and bacterial genotype of group A streptococcal (GAS) meningitis in the Netherlands.
  • To identify risk factors and outcomes associated with GAS meningitis.

Main Methods:

  • Nationwide cohort study of adult community-acquired bacterial meningitis cases in the Netherlands (2006-2013).
  • Assessed incidence, clinical presentation, complications, treatment, and outcomes.
  • Bacterial genotyping of GAS isolates was performed.

Main Results:

  • GAS meningitis accounted for 2% (26/1322) of community-acquired bacterial meningitis cases.
  • Otitis or sinusitis preceded most cases (92%). Complications were frequent (73%), including subdural empyema (35%).
  • Mortality was 19%, and 52% of survivors had neurologic sequelae. emm1 and cc28 genotypes were associated with subdural empyema (P=0.037).

Conclusions:

  • Group A Streptococcus (GAS) meningitis is an uncommon yet severe condition.
  • Subdural empyema is a significant complication, particularly associated with emm1 and cc28 GAS genotypes.
