Background:

  • The European Association of Urology Guideline Panel for Renal Cell Carcinoma (RCC) previously issued guidelines in 2010.
  • An update was necessary to reflect advancements in RCC management.

Purpose of the Study:

  • To update the 2010 RCC guideline using a robust, transparent, reproducible, and reliable methodology.
  • To provide current, evidence-based recommendations for RCC management.

Main Methods:

  • Systematic reviews adhering to Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines were conducted for prioritized topics.
  • Databases searched included Medline, Cochrane Library, trial registries, and conference proceedings from January 2000 to November 2013.
  • Risk of bias assessment and qualitative/quantitative synthesis of evidence were performed.

Main Results:

  • 151 studies involving 78,792 patients were included after reviewing 10,862 articles.
  • Randomized controlled trials (RCTs) generally had low risk of bias, but heterogeneity limited data pooling.
  • Recommendations for systemic treatment of metastatic RCC were based on higher levels of evidence due to available RCTs.

Conclusions:

  • The 2014 guideline update was performed by a multidisciplinary panel adhering to high methodological standards.
  • It provides the most reliable contemporary evidence base for managing kidney cancer.
  • International standards for kidney cancer patient care have been established.
