Background:

Gestational trophoblastic disease (GTD) encompasses a spectrum of placental-related tumors.
Recurrence of GTD after initial treatment can occur, necessitating careful monitoring.
Hydatidiform mole is the most common form of GTD.

Observation:

A 24-year-old female with a history of multiple pregnancy complications presented with a new hydatidiform mole.
Initial treatment included uterine curettage and low-dose methotrexate, leading to temporary normalization of human chorionic gonadotropin (HCG) levels.
Despite an intrauterine device, HCG positivity recurred a year later, leading to a hysterectomy.

Findings:

Post-surgical diagnosis confirmed an invasive mole, suggesting a resurgence after a period of dormancy.
The recurrence occurred independently of further chemotherapy or intercurrent pregnancy, challenging typical GTD progression models.
The case highlights the potential for delayed or 'resurrected' invasive mole development.

Implications:

This case suggests that invasive mole transformation can have a dormant phase before clinical resurgence.
Management strategies for GTD may need to consider the possibility of late recurrences.
Further research into the biological mechanisms underlying dormant GTD is warranted.
