The pineal gland, a diminutive endocrine structure named for its pinecone-shaped appearance, is situated atop the third ventricle within the diencephalon region of the forebrain. Primary optic chiasm germinomas are rare intracranial tumors. This case report details a successful treatment of a 25-year-old woman with an optic chiasm germinoma, showing no detectable tumor after treatment.

  • Intracranial germinomas commonly occur in the suprasellar and pineal regions.
  • Germinomas originating in the optic nerve or chiasm are exceptionally rare.

Observation:

  • A 25-year-old woman presented with an exophytic primary optic chiasm germinoma.
  • The patient underwent a partial tumor resection followed by radiation therapy.

Findings:

  • Magnetic resonance imaging (MRI) at 1-year follow-up revealed no detectable tumor.
  • The treatment strategy was effective in achieving complete tumor remission.

Implications:

  • This case highlights a rare presentation of intracranial germinoma.
  • Successful management of optic chiasm germinoma can be achieved with multimodal therapy.
  • Further research into rare germinoma locations is warranted.
