The Thyroid Gland

The thyroid gland is a small, butterfly-shaped gland located in the neck and covers the anterior surface of the trachea. The gland has two lateral lobes connected by a thin tissue mass called the isthmus. Internally, each lobe comprises many small spherical structures known as thyroid follicles, surrounded by a network of blood vessels.

Summary

Post-thyroidectomy bleeding (PTB) is a rare but serious complication. Unselected outpatient thyroid surgery (OTS) is not safe, requiring at least 6-hour observation and extended postoperative stay.

Background:

Thyroid operations are common in Denmark, with post-thyroidectomy bleeding (PTB) being a significant concern for outpatient thyroid surgery (OTS).
PTB can lead to tracheal compression and respiratory compromise, posing a life-threatening risk.

Purpose of the Study:

To determine the incidence and timing of PTB after thyroid surgery.
To identify risk factors associated with PTB.
To evaluate the safety of outpatient thyroid surgery (OTS).

Main Methods:

Retrospective review of data from Aarhus University Hospital (2001-2013).
Analysis of medical records for patients experiencing PTB.
Multivariate logistic regression to identify risk factors.

Main Results:

A total of 42 patients (2.8%) required re-operation due to PTB.
Male gender (OR=1.85) and thyrotoxicosis (OR=2.68) were identified as risk factors for PTB.
PTB occurred within 6 hours in 63% of cases, 6-24 hours in 25%, and >24 hours in 13%.

Conclusions:

While rare, PTB is a potentially life-threatening complication.
Unselected routine OTS is not considered safe due to the risk of PTB.
Recommend a minimum 6-hour observation period post-surgery and a 16-24 hour hospital stay for OTS patients.
