Background:

  • Osteosarcoma is the most common primary malignant bone tumor, with an incidence of 2 per million annually.
  • Orbital metastases from osteosarcoma are exceptionally rare, with only five prior cases documented in medical literature.

Observation:

  • A 19-year-old male with a history of osteosarcoma presented with acute vision loss and proptosis in the left eye.
  • Imaging revealed an orbital mass with erosion of orbital walls and intracranial invasion.
  • Surgical debulking via superotemporal orbitotomy was performed.

Findings:

  • Histopathological examination confirmed metastatic osteosarcoma with significant tumor necrosis.
  • The patient received adjuvant chemotherapy and palliative radiotherapy for symptom management.

Implications:

  • Despite its rarity, a recent increasing trend in orbital osteosarcoma metastasis necessitates heightened clinical awareness.
  • Multidisciplinary collaboration among ophthalmologists, orthopedic surgeons, and oncologists is essential for optimal patient palliation and care.
