Summary

Multi-reoperations for lung metastases can improve survival after initial procedures. However, subsequent surgeries show diminishing returns, with increased recurrence and reduced survival rates.

Area of Science:

  • Thoracic Surgery
  • Surgical Oncology

Background:

  • Lung metastases present a significant challenge in cancer management.
  • Repeated surgical intervention (metastasectomy) is a therapeutic option for selected patients.

Purpose of the Study:

  • To evaluate the role and outcomes of multiple reoperations for lung metastases.
  • To determine the prognostic factors influencing survival after repeated lung metastasectomy.

Main Methods:

  • Retrospective analysis of 113 patients undergoing repeated lung metastasectomy between 1986 and 2010.
  • Curative intent surgery with detailed recording of patient demographics, procedure numbers, and survival data.

Main Results:

  • No perioperative mortality was observed across all procedures.

  • Cumulative 5-year survival was 65% for repeated metastasectomies, significantly higher than 42% for single metastasectomy (p=0.021).
  • Increased number of resections correlated with larger tumor size, higher recurrence probability, and shorter disease-free intervals.
  • Conclusions:

    • Redo metastasectomy is beneficial for initial procedures, offering improved survival.
    • Subsequent surgeries yield progressively lower disease-free and overall survival rates.
    • Short disease-free intervals and multiple metastases are key negative prognosticators for repeated metastasectomy efficacy.
