Summary

Patients with normocalcemic primary hyperparathyroidism (NPHPT) have a higher risk of hypertension. Early intervention may be necessary even with normal calcium levels to manage parathyroid hormone (PTH) and blood pressure.

Background:

  • Primary hyperparathyroidism (PHPT) is linked to increased hypertension rates.
  • Data on hypertension in normocalcemic PHPT (NPHPT) is limited.

Purpose of the Study:

  • To investigate the association between NPHPT and blood pressure levels.
  • To determine if NPHPT increases hypertension risk.

Main Methods:

  • Retrospective study of 940 patients with measured parathyroid hormone (PTH) and calcium.
  • NPHPT group (11 patients) compared to a control group with normal PTH and calcium (296 patients).
  • Recorded systolic blood pressure (SBP), diastolic blood pressure (DBP), PTH, and calcium levels.

Main Results:

  • NPHPT patients exhibited significantly higher SBP and DBP compared to controls.
  • Adjusted analysis revealed increased risks for both SBP and DBP in NPHPT patients.
  • No significant differences in age, sex, BMI, or other metabolic markers were observed.

Conclusions:

  • NPHPT is associated with an elevated risk of high blood pressure.
  • Consideration of aggressive therapeutic interventions to normalize PTH in NPHPT is warranted, even with normocalcemia.
  • Further research into the cardiovascular implications of NPHPT is suggested.
