Monitoring Both Arms:
Monitoring BP in both arms during the initial assessment is advisable, as the systolic value may differ by five to ten mm Hg between arms. For subsequent BP assessments, use the arm with the higher reading.

Background:

  • Recurrent unexplained syncope is a common clinical problem.
  • The head-up tilt test (HUTT) is a key diagnostic tool for syncope.
  • Early hemodynamic predictors of HUTT outcome are not well-established.

Purpose of the Study:

  • To identify early hemodynamic predictors of HUTT outcome.
  • To evaluate stroke volume (SV), cardiac output (CO), and total peripheral resistance (TPR) as predictors.
  • To assess the predictive value of these parameters in healthy patients with recurrent syncope.

Main Methods:

  • Ninety-five patients with unexplained syncope underwent a nitroglycerin-potentiated HUTT.
  • Finger arterial blood pressure was recorded, and SV, CO, and TPR were computed.
  • Receiver-operating characteristic (ROC) curve analysis was used to assess predictive values.

Main Results:

  • The HUTT-positive group showed a significant heart rate increase post-nitroglycerin.
  • A significant decrease in SV and CO, and an increase in TPR were observed in the HUTT-positive group.
  • A statistically significant decrease in SV (<67 mL) strongly predicted a positive HUTT response (AUC, 0.985).

Conclusions:

  • A significant decrease in stroke volume 2 minutes after nitroglycerin administration is a strong predictor of HUTT-positive response.
  • This finding aids in the early identification of patients likely to have a positive HUTT.
  • Hemodynamic monitoring during HUTT can provide valuable diagnostic information for unexplained syncope.
