Summary

Older women undergoing breast reconstruction, especially autologous reconstruction, achieve outcomes as good as younger patients but are less likely to be offered these advanced oncoplastic and reconstructive surgical techniques.

Area of Science:

  • Oncology
  • Plastic Surgery
  • Geriatric Medicine

Background:

  • UK breast cancer guidelines recommend equivalent treatment for older and younger patients.
  • Oncoplastic and reconstructive surgical techniques are increasingly available for breast cancer patients.
  • A literature review was conducted to assess the use of these techniques in elderly patients.

Purpose of the Study:

  • To review the literature on oncoplastic and reconstructive surgery in elderly breast cancer patients.
  • To investigate the outcomes and influencing factors for these techniques in older populations.

Main Methods:

  • A MEDLINE search was performed.
  • Studies focusing on oncoplastic and reconstructive surgery in the elderly were identified and included.

Main Results:

  • Nine studies were included in the review.
  • Older patients, particularly those receiving autologous reconstruction, demonstrated outcomes comparable to younger patients.
  • Elderly patients were significantly less likely to be offered these reconstructive options.

Conclusions:

  • Despite comparable outcomes, older patients are underutilizing oncoplastic and reconstructive surgery.
  • Barriers include lack of care pathways, perceived operative risks, limited outcome data, and patient engagement.
  • Older women who opt for reconstruction report satisfaction and good quality of life, with safe options available as alternatives to mastectomy.
