Total endoscopic thyroidectomy (TET) is a safe and feasible surgical option for papillary thyroid microcarcinoma, with outcomes comparable to conventional open thyroidectomy (COT) regarding complications, despite longer operative times.

Area of Science:

  • Endocrinology
  • Surgical Oncology
  • Minimally Invasive Surgery

Background:

  • Papillary thyroid microcarcinoma presents unique surgical challenges.
  • Conventional open thyroidectomy (COT) is the standard treatment.
  • Total endoscopic thyroidectomy (TET) offers a minimally invasive alternative.

Purpose of the Study:

  • To compare the safety and efficacy of TET versus COT for papillary thyroid microcarcinoma.
  • To evaluate short-term clinical outcomes, including operative time, complications, and hospital stay.

Main Methods:

  • A meta-analysis of nonrandomized comparative studies published between January 1996 and July 2014.
  • Inclusion of studies comparing TET and COT for papillary thyroid microcarcinoma.
  • Pooled analysis of perioperative outcomes using weighted mean differences and odds ratios.

Main Results:

  • Five studies with 1004 patients (475 TET, 529 COT) were included.
  • TET was associated with significantly longer operative times (WMD, 48.15 minutes).
  • TET involved fewer removed lymph nodes (WMD, -0.68) but showed no significant differences in hospital stay or rates of recurrent laryngeal nerve palsy and hypocalcemia.

Conclusions:

  • Total endoscopic thyroidectomy is a feasible and safe surgical procedure for selected patients with papillary thyroid microcarcinoma.
  • TET demonstrates comparable short-term safety outcomes to conventional open thyroidectomy.
  • Further research may explore long-term outcomes and patient selection criteria for TET.
