Summary

A history of headaches and high intracranial pressure, not fear, are key risk factors for post-lumbar puncture (LP) headache. Understanding these factors can help predict and potentially prevent this common complication.

Background:

  • Post-lumbar puncture (LP) headache affects approximately one-third of patients.
  • Identifying risk factors for post-LP headache is crucial for patient care.

Purpose of the Study:

  • To evaluate potential risk factors associated with the development of post-LP headache.
  • To investigate the correlation between patient anxiety and post-LP headache occurrence.

Main Methods:

  • 144 patients undergoing diagnostic LP were surveyed regarding pre-existing headache, fear of the procedure, and post-LP headache.
  • Patient demographics, operator experience, and procedure-related data were collected.
  • Statistical analysis compared characteristics of patients with and without post-LP headache.

Main Results:

  • Post-LP headache occurred in 27.6% of patients.
  • Younger age and higher proportion of women were noted in the headache group.
  • A history of headaches (p=0.003) and high opening pressure (p=0.078) were significantly associated with post-LP headache.
  • Fear of the procedure did not correlate with post-LP headache occurrence.

Conclusions:

  • A history of headache and elevated intracranial pressure are significant risk factors for post-LP headache.
  • Patient anxiety or fear of the lumbar puncture procedure is not a predisposing factor.
  • Findings suggest pathophysiology involving changes in compliance and pressure gradients.
