Background:

Periocular injuries from dog bites are common in children.
Intraocular trauma from canine attacks is exceptionally rare.

Observation:

A 7-year-old boy experienced a zone 3 ruptured globe injury from a Perro de Presa Canario attack.
Initial visual acuity was severely impaired (counting fingers).

Findings:

Surgical repair addressed a 15 mm inferotemporal corneoscleral laceration with uveal prolapse.
Postoperative visual acuity improved to 20/40 within one month.

Implications:

This case highlights the potential for severe intraocular damage from dog bites.
Early surgical intervention is crucial for preserving vision in such pediatric cases.
