Measurement of pacing threshold.

H Ector, E Witters, L Tanghe

Pacing and Clinical Electrophysiology : PACE

    PubMed
    Summary

This study reports pacing thresholds for Cardiac Pacemakers, Inc. (CPI) 4118 tined leads, finding low constant current (≤0.6 mA) and constant voltage (≤0.5 V) thresholds in most cases. No definite difference was found between pacing loss and reestablishment thresholds.

    Area of Science:

    • Biomedical Engineering
    • Cardiovascular Devices
    • Electrophysiology

    Background:

    • Cardiac pacing relies on tined leads to deliver electrical stimuli.
    • Accurate threshold determination is crucial for effective pacemaker function and patient safety.
    • The CPI 4118 tined lead is a commonly used device in cardiac pacing.

    Purpose of the Study:

    • To report the constant current and constant voltage thresholds for the CPI 4118 tined lead.
    • To evaluate external pacemakers and pacing system analyzers as threshold testing devices.
    • To investigate potential differences in pacing thresholds during stimulus reduction versus increase.

    Main Methods:

    • Thresholds were determined for CPI 4118 tined leads using both constant current and constant voltage modes.
    • Three external pacemakers and two pacing system analyzers were utilized for threshold testing.
    • A calibrated isolation amplifier was employed for analyzing stimulus current and voltage waveforms.

    Main Results:

    • In 98% of cases, constant current thresholds were ≤0.6 mA.
    • In 96% of cases, constant voltage thresholds were ≤0.5 V.
    • No significant difference was observed between pacing loss and reestablishment thresholds.

    Conclusions:

    • The CPI 4118 tined lead demonstrates low and consistent pacing thresholds.
    • The evaluated devices are suitable for threshold testing of these leads.
    • Further investigation may be needed to clarify the pacing loss and reestablishment threshold behavior.
