Osteotome sinus floor elevation (OSFE) without grafting achieved 100% implant survival over 10 years in the atrophic maxilla. This predictable procedure results in significant endo-sinus bone gain, demonstrating long-term efficacy.

Background:

  • Long-term outcomes of osteotome sinus floor elevation (OSFE) without grafting in the atrophic maxilla are not well-established.
  • Understanding the predictability of this technique is crucial for clinical decision-making.

Purpose of the Study:

  • To evaluate the long-term efficacy and peri-implant bone stability after OSFE without grafting.
  • To assess the predictability of implant placement in resorbed posterior maxilla using this technique.

Main Methods:

  • Twenty-five implants (≤10mm) were placed in 17 patients using OSFE without grafting in atrophic maxilla (mean residual bone height: 5.4mm).
  • Bone levels were monitored using periapical radiographs at 1, 3, 5, and 10 years post-surgery.

Main Results:

  • 100% implant survival rate observed at 10 years (23 implants in 15 patients).
  • Significant endo-sinus bone gain (mean: 3.0mm) and limited crestal bone loss (mean: 1.0mm) were recorded.
  • Bone gain primarily occurred within the first year, with stable bone levels thereafter.

Conclusions:

  • OSFE without grafting is a predictable and highly successful procedure for implant placement in the atrophic maxilla.
  • The technique demonstrates excellent long-term implant survival and stable peri-implant bone levels.
  • Endo-sinus bone augmentation is mainly achieved during the initial year following the procedure.
