Isolated cortical vein thrombosis (ICVT) is rare but can cause severe brain damage. Early diagnosis using MRI, particularly susceptibility-weighted imaging, is crucial for effective treatment with anticoagulation therapy.

- Isolated cortical vein thrombosis (ICVT) is an uncommon cause of cerebral infarction, accounting for less than 1% of cases.

- To present clinical and radiological findings in 7 patients with ICVT.
- To identify key risk factors and optimal imaging modalities for timely ICVT diagnosis.

- Adult females were the majority, aligning with existing literature.
- Hypercoagulable states and intracranial hypotension were significant risk factors.
- Intraparenchymal involvement was common, manifesting as edema, hemorrhage, or infarction. Susceptibility-weighted imaging demonstrated the highest sensitivity for ICVT detection.
