Summary

Robot-assisted surgery offers a safe and effective palliative option for advanced malignant biliary obstruction. This minimally invasive approach resulted in a low complication rate and improved patient quality of life.

Purpose of the Study:

  • To evaluate the efficacy and safety of robot-assisted laparoscopic hepaticojejunostomy for palliative treatment of advanced malignant biliary obstruction.

Main Methods:

  • A retrospective review of nine patients who underwent robot-assisted laparoscopic hepaticojejunostomy between May 2009 and April 2014.
  • Procedures included Roux-en-Y hepaticojejunostomy and double bypass (hepaticojejunostomy and gastrojejunostomy).

Main Results:

  • Successful completion of robot-assisted hepaticojejunostomy in all nine patients.
  • Mean operating time: 212.8 minutes; mean blood loss: 38.7 mL.
  • Overall complication rate: 22.2% (one bile leak); no procedure-related mortality. Mean survival: 11.1 months.

Conclusions:

  • Robot-assisted laparoscopic hepaticojejunostomy is a feasible and safe palliative procedure for malignant biliary obstruction.
  • The approach is associated with a low complication rate and potentially improved quality of life.
