Background:

  • Traditional word-finding tasks are common in neuropsychological evaluations for older adults.
  • A significant limitation exists as many older adults have visual impairments, and no word-finding tasks are specifically designed for this population.
  • Dysnomia, or word-finding difficulty, is a concern in older adults.

Purpose of the Study:

  • To develop and validate a verbal, non-visual word-finding measure for older adults, particularly those with visual impairments.
  • To assess the measure's reliability, validity, and diagnostic accuracy for detecting dysnomia in older Veterans.

Main Methods:

  • A 60-item verbal scale using everyday spoken language stimuli was created.
  • The scale was administered to 131 older Veterans.
  • Rasch analyses and differential item functioning were used to refine the scale to 55 items. Concurrent validity was assessed using established neuropsychological tests.

Main Results:

  • The final 55-item scale demonstrated good internal consistency (coefficient alpha = 0.84).
  • The scale showed strong correlations with the Neuropsychological Assessment Battery Naming test (r=0.84) and moderate correlations with Delis-Kaplan Executive Function System (D-KEFS) fluency measures.
  • Receiver Operating Characteristic (ROC) analyses indicated high sensitivity (79%) and specificity (85%) for detecting dysnomia. Patients with dysnomia performed significantly worse than those with intact word-finding.

Conclusions:

  • The new verbal, non-visual word-finding measure is a promising tool for the neuropsychological evaluation of word-finding ability in older adults, accommodating those with and without visual impairment.
  • The measure effectively differentiates between individuals with and without dysnomia and shows potential for distinguishing cognitive impairment levels.
  • Future research should focus on creating a shorter version and expanding normative data.
