Plasma osteopontin (OPN) levels are significantly elevated in acute liver failure (ALF), correlating with liver damage severity. Very high OPN may indicate better outcomes in ALF patients.

- ALF patients exhibited 10- to 30-fold higher median plasma OPN levels (1055 ng/mL) compared to healthy controls (41 ng/mL).
- OPN levels were highest in acetaminophen- and ischemia-related ALF, correlating with aminotransferase levels (R=0.47 for AST, p<0.001).
- Elevated OPN levels were also observed in rheumatoid arthritis and post-spine fusion surgery patients, but were lower than in ALF.
Conclusions:
- Plasma OPN levels correlate with the degree of liver necrosis in ALF.
- Extremely high OPN levels were associated with hyperacute liver injury and favorable outcomes.
- The potential protective role of OPN in mitigating ALF progression requires further investigation.
