    Summary

    Generalized aspergillosis and endocarditis were found in a patient with fever of unknown origin. A transvenous pacemaker lead may have been the initial infection site, highlighting risks of cardiac pacing.

    Background:

    • A 65-year-old woman presented with FUO.
    • Autopsy revealed generalized aspergillosis and endocarditis.
    • A transvenous pacemaker lead, in situ for 2 years, was encased in an infected thrombus.

    Findings:

    • The pacemaker lead was a potential primary site for invasive fungal infection.
    • Aspergillosis and endocarditis developed in a patient without typical risk factors.
    • Septicemia secondary to endocarditis was confirmed.

    Implications:

    • Cardiac pacing complications, including fungal endocarditis, must be considered in FUO evaluations.
    • Early recognition and management of pacemaker-related infections are crucial.
    • This case underscores the importance of considering rare pathogens in immunocompetent patients with indwelling devices.
