Cytologic changes simulating malignancy in thyrotoxic goiters treated with carbimazole.

V Smejkal, E Smejkalová, M Rosa

    Summary

Cytologic examination of toxic goiters treated with carbimazole revealed nuclear changes mimicking malignancy. However, toluidine blue staining of nucleolar morphology confirmed benign cell types, preventing diagnostic errors.

    Area of Science:

    • Endocrinology
    • Cytopathology
    • Oncology

    Background:

    • Toxic goiters can present cytologic atypia, including enlarged nuclei, potentially misdiagnosed as malignancy.
    • Carbimazole is a common treatment for toxic goiters, and its cellular effects require careful cytologic evaluation.

    Purpose of the Study:

    • To investigate the cytologic patterns in toxic goiters treated with carbimazole.
    • To differentiate between treatment-induced nuclear changes and true malignancy using specific staining techniques.

    Main Methods:

    • Cytologic analysis of panoptic-stained material from patients with toxic goiters treated with carbimazole.
    • Morphologic study of nucleolar characteristics using toluidine blue staining.

    Main Results:

    Related Experiment Videos

    • Panoptic staining showed unusually large nuclei, resembling malignant cells.
    • Toluidine blue staining revealed nucleolar morphology consistent with benign euthyroid colloid goiters, not malignant types.
    • No evidence of increased nuclear transcription activity was observed.

    Conclusions:

    • Nucleolar morphology assessment via toluidine blue staining is crucial for avoiding misdiagnosis of malignancy in carbimazole-treated toxic goiters.
    • Carbimazole appears to exert a cytostatic effect on thyroid cells, consistent with existing literature.
