[Transabdominal oocyte retrieval: case report on an IVF cycle]

Background:

• Transvaginal ultrasound-guided follicular aspiration is the standard for oocyte retrieval in IVF.
• Difficult ovarian access can compromise standard oocyte retrieval.
• Alternative methods are necessary when the transvaginal approach is not feasible.

Observation:

• A 36-year-old patient with primary infertility and bilateral tubal occlusion underwent IVF.
• Ovarian stimulation was performed.
• The left ovary was accessible via transvaginal aspiration, while the right ovary was adhered to the anterior uterine wall, necessitating a transabdominal approach.

Findings:

• A mixed follicular aspiration was performed, utilizing both transvaginal and transabdominal ultrasound guidance.
• Three oocytes at metaphase II were successfully retrieved from the right ovary using the transabdominal technique.
• The procedure was completed without complications.

Implications:

• Ultrasound-guided transabdominal follicular aspiration is a viable and effective alternative for oocyte retrieval in cases of compromised transvaginal access.
• This technique expands the possibilities for successful IVF treatment in patients with challenging pelvic anatomy.
• Successful oocyte retrieval using this method contributed to the patient achieving pregnancy.
