Background:

  • Existing inframammary fold (IMF) reconstruction techniques have limitations.
  • Accurate IMF positioning is challenging, especially without implants.
  • Physiological fixation of the IMF to the chest wall is difficult to achieve.

Purpose of the Study:

  • To introduce and evaluate a novel drawstring method for inframammary fold (IMF) reconstruction.
  • To assess the efficacy and safety of the barbed suture technique for IMF creation.
  • To determine the adjustability and aesthetic outcomes of the new IMF reconstruction approach.

Main Methods:

  • A barbed suture is used to recreate the IMF by drawing through the dermis.
  • The suture is anchored to the costal cartilage medially and secured externally laterally.
  • IMF level and depth are finely adjusted via suture tension post-implant insertion.

Main Results:

  • The drawstring method was applied in 102 patients (95 breast implants, 7 flaps).
  • No suture or implant infection or exposure occurred.
  • Suture slack, leading to a shallow IMF, was observed in 10 patients, linked to insufficient skin expansion and strong caudal traction.

Conclusions:

  • The drawstring method effectively creates smooth and symmetrical inframammary folds (IMFs).
  • Sufficient skin expansion is crucial for preventing IMF slack.
  • This technique offers a viable option for IMF reconstruction with a low revision rate of 2.9%.
