Endoscopic Studies I: Bronchoscopy and Thoracoscopy

993
Endoscopy is a non-surgical medical technique used to examine a person's internal organs and vessels. This lesson will focus on two types of endoscopic studies: bronchoscopy and thoracoscopy.
Bronchoscopy
Description
Bronchoscopy is a procedure that involves direct visualization of the larynx, trachea, and bronchi for diagnostic and therapeutic purposes. A flexible fiber optic or rigid bronchoscope is used to carry out the procedure. The fiber-optic bronchoscope is more frequently used due...

Summary

For transaxillary thyroidectomies, endoscopic surgery showed a faster learning curve and quicker procedure times than robotic surgery in initial low-volume center experiences. Both methods yielded comparable outcomes.

Area of Science:

  • Minimally Invasive Surgery
  • Endocrine Surgery
  • Surgical Technology

Background:

  • Transaxillary thyroidectomy is gaining acceptance, with high-volume centers publishing their experiences.
  • Low- to moderate-volume centers require practical guidance for adopting transaxillary approaches.

Purpose of the Study:

  • To compare the initial experience and practicality of robotic versus endoscopic transaxillary thyroidectomy for centers with limited experience.
  • To assess operative times, learning curves, and surgeon perceptions.

Main Methods:

  • A retrospective case series of 101 patients undergoing transaxillary thyroidectomy over 4 years.
  • 48 patients had robotic thyroidectomy, and 53 had endoscopic thyroidectomy.
  • Data included patient characteristics, procedure duration, pathology, complications, and a surgeon survey.

Main Results:

  • Endoscopic hemithyroidectomy was significantly shorter (145.8 min) than robotic (193.6 min).
  • The endoscopic group showed a greater improvement in operative time over the first 10 cases (49.1%) compared to the robotic group (18.6%).
  • Transient recurrent laryngeal nerve injury occurred in 2 cases; surgeons preferred robotic for its learning curve but endoscopic for less need for support.

Conclusions:

  • Both robotic and endoscopic transaxillary thyroidectomy are comparable in initial outcomes.
  • The endoscopic technique may have a steeper learning curve but potentially faster steady-state procedure times.
  • Findings are relevant for low- to moderate-volume centers initiating transaxillary thyroidectomy programs.
