Summary

Repeat liver stereotactic body radiation therapy (SBRT) is safe and can induce liver regeneration. Compensatory hypertrophy in the untreated lobe maintained overall liver volume in a breast cancer patient.

Background:

  • Liver stereotactic body radiation therapy (SBRT) is an emerging treatment for oligometastases.
  • SBRT may offer survival benefits in select patients with liver metastases.

Purpose of the Study:

  • To document the first case of liver regeneration (LR) after repeat liver SBRT.
  • To assess the safety and efficacy of repeat liver SBRT in a patient with breast cancer metastases.

Main Methods:

  • The study involved a single patient with breast cancer metastatic to the liver.
  • Repeat SBRT was delivered to the right hepatic lobe.
  • Liver volume changes were monitored before and after treatment.

Main Results:

  • Significant retraction of the treated right lobe (approx. 50% volume reduction) was observed.
  • Compensatory hypertrophy of the contralateral lobe occurred (320% volume increase).
  • Overall liver volume remained stable within ±5% of baseline.

Conclusions:

  • Repeat liver SBRT can be delivered safely.
  • Compensatory contralateral lobe hypertrophy effectively maintains functional liver volume after repeat SBRT.
