Ankle Joint01:10

Ankle Joint

3.8K
The ankle is formed by the talocrural joint (crural = leg). It consists of the articulations between the talus bone of the foot and the distal ends of the tibia and fibula of the leg. The superior aspect of the talus bone is square-shaped and has three areas of articulation. The top of the talus articulates with the inferior tibia. This is the portion of the ankle joint that carries the body weight between the leg and foot. The sides of the talus are firmly held in position by the articulations...

Summary
This summary is machine-generated.

Total ankle replacement offers good to very good long-term outcomes for end-stage ankle arthritis, restoring function and mobility. However, patient age and osteoarthritis type are key factors influencing success rates.

Area of Science:

  • Orthopedic surgery
  • Rheumatology
  • Biomedical engineering

Background:

  • Ankle osteoarthritis affects approximately 1% of adults, causing significant pain.
  • Limited data exists on the long-term efficacy of conservative treatments for ankle osteoarthritis.
  • Non-joint-preserving surgical options like total ankle replacement and ankle fusion are available for advanced cases.

Purpose of the Study:

  • To review the current literature and clinical experience regarding total ankle replacement.
  • To evaluate the outcomes and risk factors associated with total ankle replacement for end-stage ankle arthritis.

Main Methods:

  • A review of selected relevant publications and guidelines.
  • Inclusion of authors' personal clinical experience.
  • Analysis of existing data on total ankle replacement outcomes and failure predictors.

Main Results:

  • Total ankle replacement with modern prostheses demonstrates high success rates (up to 90% at 10 years).
  • Independent risk factors for failure include age over 70, primary or post-traumatic osteoarthritis, and specific prosthesis types.
  • Restored range of motion averages 25-30°, with potential up to 60°.

Conclusions:

  • Total ankle replacement is a viable treatment for end-stage ankle arthritis, improving function and pain.
  • Contraindications must be carefully considered.
  • Further research on biomechanics and long-term follow-up is warranted.
