Purpose of the Study:

  • To assess phagocytic function using chemiluminescence in type 2 diabetic patients undergoing glycemic control improvement.
  • To compare phagocytic function between poorly controlled type 2 diabetic patients with improving glycemic control and well-controlled type 1 diabetic patients.

Main Methods:

  • Serial whole blood chemiluminescence measurements were performed.
  • Phagocytic function was assessed by corrected chemiluminescence, normalized to polymorphonuclear cell count.

  • Glycemic control was monitored, with changes in HbA1c (delta HbA1) analyzed.
  • Main Results:

    • Chemiluminescence remained below normal in most type 2 patients initially.
    • Significantly increased chemiluminescence was observed in type 2 patients with improved glycemic control after 12 weeks (0.25 photons/second/cell) compared to those without improvement (0.12 photons/second/cell).
    • A significant inverse correlation was found between changes in HbA1c and changes in chemiluminescence (delta HbA1 with delta chemiluminescence). Type 1 diabetic patients showed values within the normal range but with wide scatter.

    Conclusions:

    • Glycemic control is a primary determinant of phagocytic function in diabetic patients.
    • While improved glycemic control enhances phagocytic function in type 2 diabetes, other factors likely influence this process, particularly in type 1 diabetes.
