Background:

Parathyroid cysts are uncommon neck lesions, rarely discussed in otolaryngology literature.
They originate from pharyngeal pouch remnants or cystic degeneration of parathyroid adenomas.

Observation:

A case report details a parathyroid cyst in a 13-year-old female, the youngest reported patient.
Fine needle aspiration (FNA) of an anterior neck mass revealed clear fluid, characteristic of a parathyroid cyst.

Findings:

Radioimmunoassay confirmed the diagnosis by detecting high parathormone and low thyroglobulin levels in the aspirated fluid.
This FNA diagnosis obviated the need for empirical thyroid hormone suppression or hemithyroidectomy.

Implications:

Fine needle aspiration is a valuable diagnostic tool for parathyroid cysts.
Early and accurate diagnosis can prevent unnecessary surgical interventions for neck masses.
