Apicoectomy showed better apical repair than apexification for non-vital front teeth with large radiolucencies. This study compared two endodontic procedures in young patients following dental trauma.

Area of Science:

  • Endodontics
  • Pediatric Dentistry
  • Dental Traumatology

Background:

  • Evaluating treatment outcomes for non-vital maxillary central incisors in young patients.
  • Assessing management of adjacent teeth with large periapical radiolucencies post-trauma.

Observation:

  • A 10-year-old boy with trauma-induced non-vital central incisors (11 and 21) presented with tenderness.
  • One incisor underwent apexification (Metapex), the other apicoectomy (gutta-percha).
  • Radiographical follow-up occurred at 6 months, 1 year, and 2 years.

Findings:

  • Apicoectomy demonstrated more favorable apical repair compared to apexification.
  • This outcome was observed in a non-vital maxillary central incisor with an open apex and significant periapical radiolucency.

Implications:

  • Apicoectomy may be a preferred treatment for specific cases of immature non-vital teeth with extensive periapical pathology.
  • Findings inform clinical decision-making in pediatric endodontics and dental trauma management.
