Researchers identified a three-gene signature to accurately diagnose papillary thyroid carcinoma (PTC) and assess metastasis risk. This molecular tool could reduce unnecessary surgeries for thyroid nodules.
Area of Science:
- Oncology
- Molecular Biology
- Genomics
Background:
- Thyroid nodules are prevalent, with papillary thyroid carcinoma (PTC) being the most common malignancy.
- The natural progression of PTC is not well understood, and current diagnostic limitations lead to avoidable surgeries.
Purpose of the Study:
- To discover novel molecular markers for improved diagnosis of thyroid lesions.
- To develop a diagnostic algorithm for differentiating thyroid tumors.
Main Methods:
- Gene expression profiling via microarray on 61 PTC and 13 normal thyroid tissues.
- Cross-study validation using external datasets (138 PTC/normal pairs).
- Development and validation of a diagnostic algorithm using RT-qPCR on 28 transcripts, including benign thyroid lesions (BTL) and other follicular cell-derived thyroid carcinomas (OFDTC).
Main Results:
- GABRB2 identified as the most consistently upregulated gene in PTC.
- PTC molecular alterations suggest impaired T4 responsiveness and altered retinoic acid metabolism.
- A three-transcript algorithm (CLDN10, HMGA2, LAMB3) achieved 94% sensitivity and 96% specificity in distinguishing PTC from BTL.
- Higher algorithm scores correlated with increased risk of regional lymph node metastasis.
Conclusions:
- A highly accurate diagnostic tool for PTC based on three key transcripts has been developed.
- This molecular signature shows potential for predicting lymph node metastasis in PTC patients.
- The findings may lead to more precise management of thyroid nodules and reduce overtreatment.
