    Subclavian vascular catheters (SVC) provide safe temporary vascular access for hemodialysis. These catheters are suitable for end-stage renal failure patients awaiting definitive access, with manageable complications.

    Background:

    • Temporary vascular access is crucial for hemodialysis.

    • End-stage renal failure (ESRF) patients often require temporary solutions while awaiting definitive access.

    Purpose of the Study:

    • To evaluate the safety and efficacy of subclavian vascular catheters (SVC) for temporary vascular access.
    • To assess complications associated with SVC use in hemodialysis patients.

    Main Methods:

    • Retrospective review of 37 consecutive patients undergoing 461 hemodialyses using 49 SVCs over 18 months (Jan 1984 - Jun 1985).
    • Analysis of patient demographics, including renal failure type (acute vs. ESRF) and treatment setting (outpatient vs. inpatient).
    • Documentation and analysis of all clinical complications, particularly infectious episodes.

    Main Results:

    • A total of 461 hemodialyses were performed using SVCs in 37 patients.
    • The primary complication was 27 episodes of clinical septicemia, with most patients responding to antibiotics and catheter removal.
    • SVCs were utilized mainly in ambulatory ESRF patients awaiting definitive vascular access.

    Conclusions:

    • Subclavian vascular catheters (SVC) are safe and effective for extended short-term vascular access.
    • SVCs are particularly suitable for ESRF patients experiencing difficulties with vascular access.
    • Septicemia was the main complication, but it was generally manageable with appropriate treatment.
