Kidney transplant recipients (KTRs) receiving organs from homozygous donors had poorer graft survival. Donor homozygosity for human leukocyte antigen (HLA)-DR and HLA-B is a risk factor for kidney transplant failure.
Area of Science:
- Nephrology
- Immunogenetics
- Transplantation
Background:
- Human leukocyte antigen (HLA) matching improves long-term renal allograft survival.
- The impact of HLA gene dosage in mismatched transplants remains unclear.
- This study investigates HLA gene dosage effects on kidney transplant outcomes.
Purpose of the Study:
- To determine the effect of donor HLA homozygosity versus heterozygosity on kidney transplant outcomes.
- To assess the association between HLA gene dosage and death-censored graft survival in kidney transplant recipients (KTRs).
Main Methods:
- Analysis of HLA typing data from 697 kidney transplant recipients (KTRs) and their donors.
- Categorization of KTRs based on whether their donors were HLA homozygous or heterozygous.
- Comparison of death-censored graft survival using Kaplan-Meier analysis and Cox proportional hazards models.
Main Results:
- Graft survival was significantly longer in KTRs with heterozygous HLA-DR and HLA-B donors compared to homozygous donors.
- HLA-DR and HLA-B donor homozygosity independently increased the risk of death-censored graft failure.
- No significant association was found between HLA-A allele states and graft survival.
Conclusions:
- HLA donor mismatch due to HLA-DR and HLA-B homozygosity significantly elevates the risk of kidney graft failure.
- Donor allele state, in addition to the number of HLA mismatches, is crucial for predicting transplant outcomes.
- Considering donor homozygosity/heterozygosity can refine risk assessment for kidney transplant recipients.

Renal Failure: Dose Adjustments
Reduced renal clearance and elimination rate are common outcomes of renal impairment. These alterations lead to a prolonged elimination half-life and an altered apparent volume of distribution for drugs. As a result, dosage adjustments are typically necessary to maintain optimal drug levels in the body.
However, dosage adjustments...
