Sharing information, concepts, and emotions to foster mutual understanding is communication. The sender, recipient, and transaction must be considered in this manner. The sender is the person who shares the message, the recipient is the person who receives and understands the message, and the transaction is the method used to deliver the message and the variables that affect the communication's context and surroundings. Communication between two animals occurs when one animal transmits an information signal that causes a change in the animal that receives the information. Organisms communicate with one another in a host of different ways. Signals can be auditory, chemical, visual, tactile, or a combination of these. Communication is a critical behavioral adaptation that promotes survival, growth, and reproduction.

Background:

Poor communication among healthcare professionals is a significant barrier to patient safety.
Existing literature primarily focuses on communication as transactional (information exchange), neglecting its transformational aspects (causing change).
This paper advocates for integrating both transactional and transformational communication paradigms within implementation science.

Purpose of the Study:

To explore how communication is conceptualized in implementation science literature.
To debate the merits of encompassing both transactional and transformational communication paradigms.

Main Methods:

A two-staged literature review was conducted to identify communication concepts in implementation science.
Twenty-seven relevant theories, models, or frameworks were identified and analyzed.

Main Results:

Most identified models conceptualize communication solely as a transactional process.
Rogers' Diffusion of Innovations theory is an exception, defining communication and including both transactional and transformational paradigms.
A narrow focus on information exchange misrepresents the complex reality of communication in healthcare.

Conclusions:

Implementation science should adopt a broader view of communication, incorporating shared understanding, multi-layered interactions, social context, and unpredictability.
Integrating transformational communication, which emphasizes shared understanding and human interaction, is crucial for successful implementation interventions.
Explicitly acknowledging and utilizing both transactional and transformational communication paradigms can enhance implementation efforts and clinical intervention adoption.
